Student Day Application Requirements

STUDENT DAY APPLICATION REQUIREMENTS
FSMTA DISTRICT IV STUDENT DAY
March 25, 2023
Please complete the requirements below and mail your Student Day application packet to:

       Angie Holt

​All materials listed below must be received on or before Saturday, February 11, 2023. Please allow
enough time for mail delivery.

  1. One copy of the completed Achievement Examination application form. This form is available here.
  2. If you are registering siblings, please indicate this on the Achievement Examination application form. Every attempt will be made to schedule in the same time block.
  3. One copy of the Judging Sheet for each student at appropriate level. Please fill in student name, check Achievement or Progress, and fill in Title, Composer, and Time in Section 2: Repertoire. These sheets are in the Student Handbook.
  4. If you know your teacher number (It is the same every year), please write it in the top right hand corner of all paperwork.
  5. One check for all fees, payable to FSMTA District IV. Individual student fees are $30 for all levels. Please add $15 to the total for your continental breakfast and lunch. 
  6. Your registration and schedule information will be returned to you by email unless you prefer receiving it by postal mail.

Detailed information concerning requirements and judging is listed in the Handbook of Student Activities. Please note any new requirements instituted this year. Members may download the entire Handbook or a portion thereof at fmta.org. You may access the Handbook with your member #.

Please adhere to guidelines when deciding your student’s entry level and repertoire, or your student may be disqualified. Student Day is open to piano, voice, string, woodwind, guitar, and percussion students. If you plan to enter students in a category other than piano, please contact me as soon as possible for additional information.
